CHAPTER 20 - METHODS OF WORK
23 - SAFETY.
23.1 - Safety Discussion at the Prework Conference.
Vegetation Management (VM) Projects. Advise contractors that Form R6-FS-210012 (a two-sided form) "Vegetation Management Reporting Form" must be completed
in two stages:
1. At the beginning of the project, initiate documentation by filling out the
project identification data at the top of side 1, employee name and assignment for
each worker, and check VM method used. For herbicide projects, fill in the
herbicide data at the bottom of side 1, and obtain employee consent signatures on
side 2. Contractor shall retain original for completion at end of project and make
photocopies to give to the Contracting Officer's Representative (COR).
2. At the end of the project, on the face of the original form, complete all
remaining data. Under the category "accident or exposure incident" either "yes" or
"no" must be checked separately for EACH worker. Original is forwarded to COR.
23.2 - Safety Responsibilities of Forest Service Employees.
For Vegetation Management Projects, ensure that Form R6-FS-2100-12 is
completed in full. If there has been an accident or exposure, ensure a separate
sheet is prepared explaining the incident. Review and promptly forward to the
Contracting Officer (CO).
24 - INSPECTIONS.
24.2 - Performing Inspections.
24.21 - Inspection Activities. Special Requirements of Vegetation Management
projects are:
1. Before a Notice to Proceed can be issued at the start of the project, conduct
an inspection to ensure that all equipment and clothing required by Chapter III of
the "Guide to Conducting Vegetation Management Projects in the Pacific
Northwest" (VM Guide) are on site.
2. Conduct periodic checks to ensure that equipment and clothing which
workers are required to use are in fact used for the duration of the project.

3. Verify that protective clothing specified by the VM Guide to be worn at the
option of the worker is on site and available for use at all times during the project.
If the contractor fails to comply, the inspector or COR shall issue Form FS-6300-12,
Work Order and Notice of Noncompliance, and advise the CO who shall determine if
further action is required.
24.21a - How to prepare.
If the contract involves vegetation management, review Chapter III of the VM
Guide for requirements related to equipment and clothing.
