51 - CONTRACTING OFFICER'S REPRESENTATIVE RECORDS.
Vegetation Management Reporting Requirements. Prior to authorizing final
payment at the end of the project, the Contracting Officer's Representative (COR)
shall receive the original of Form R6-FS-2100-12 "Vegetation Management
Reporting Form" with all required information filled in. The COR shall insure that
either "yes" or "no" has been checked separately for each worker in the space labeled
"accident or exposure incident." Retain a copy in the COR file and mail original to
the Contracting Officer (CO). CO reviews, retains a copy and forwards original to
the Regional Office Forest Pest Management-Integrated Pest Management
Specialist.
