Catholic School Learning Plan for St. Joachim Elementary School Annual Progress Report Shared Priorities: Creating Catholic Conditions for Well-Being, Learning and Leading Outcomes: 1.0 By the end of 2016, there will be an increase in proportion of students indicating that they feel accepted by the students in the school as demonstrated by the CCCC School Climate Survey. Outcome Progress Comments 6/5/2015: 1.0 Our Catholic Community Culture and Caring Action Team is in place to develop programs for creating Catholic conditions: WITS (walk away, ignore, talk it out, seek help), THINK before you speak (is it thoughtful, helpful, inspiring, necessary, kind), Peel Police presentations, PALS, Peacemakers, Boys Night In, Girls Night In. Parents and students are also invited to attend PACT evenings run by our School Social Worker and Peel Children's Centre. We are continuing to run our Breakfast Program and monthly virtues program. We listen to the voice of staff, students and parents when planning for our school community. We continue to use Jesus as our guide. Shared Priorities: Building Collaborative Practices Through Inquiry Outcomes: 2.0 By the end of 2016, there will be an increase in the number of students who have demonstrated improvement on the learning skill and work habit of collaboration. Outcome Progress Comments 6/5/2015: 2.0 Our school inquiry: If we co-create with students the learning skills success criteria in Math and focus on student engagement, student achievement will increase. Students and staff have co-created the learning skills for Math and have focused on student engagement. We are noticing that by explicitly teaching the learning skills, contributes to improvement in student engagement. Shared Priorities: Enhancing Transitional Practices Outcomes: 3.0 By the end of 2016 there will be an increase in the proportion of students who have demonstrated improvement on the learning and work habit of self-regulation as they prepare for Secondary School. Outcome Progress Comments 6/4/2015: 3.0 Our school inquiry focused on learning skills. These skills will help students with their transition as they prepare for secondary school. We are continuing to support students through effective student transitions. We are facilitating ongoing communication between home and Notre Dame Secondary School to support student transitions to the secondary panel. All students are supported with appropriate transition strategies. We have ensured all students with an IEP have an effective transition plan aligning with PPM 156. Outcome Progress Comments 6/5/2015: 4.0 We are continuing our quarterly Student Intervention meetings with our staff to ensure that we understand and responsive to the students in our school community. This, along with our school inquiry focused on learning skills, has led to an improvement in student the learning skills and achievement. Shared Priorities: Responding through Effective Instruction Outcomes: 5.0 By the end of 2016, there will be an increase in the proportion of students improving their EQAO achievement level between assessment years. Outcome Progress Comments 6/4/2015: We are listening to the voice of all learners and ensuring all students have an entry point in the learning. Staff were offered opportunities to co-plan and are providing descriptive feedback and rubric assessments to support student understanding of their own personal next steps.
