The purpose of this guide is to demonstrate how to conduct surveys with a matrix of choices in
USAonline.
1. After clicking Tests and Quizzes on the left side of the screen, create a new assessment using Assessment Builder .
2. Choose Survey- Matrix of Choices from the Add Question dropdown menu.

3. Type a question into the Question Text box (1), then type the row choices that will appear on the left-hand side of the matrix (2), as well as the column choices that will appear the top of the matrix (3). To add more rows or column choices to the matrix, press the Enter key after you type each row and column choice.

4. Click Save after you have entered the row and column choices for the matrix. You will then see how the matrix will appear to your students. You may add more questions to your survey by clicking the drop down menu next to Insert New Question .

5. After you are finished creating questions for the matrix, go to the Grading tab under
Settings . Since USAonline automatically defaults the answer point value for a survey question to 0, you will need to change the Gradebook Options in the Settings to None (1).
6. If you want to keep students’ identities anonymous, select the Anonymous Grading Only option (2). This option replaces students’ names with numbers, so you will not be able to view what a specific student has submitted.
NOTE: You can update Grading settings for your published survey after students have submitted their surveys to reveal the name for each submission. If you want to truly make your survey anonymous, you should not change the Grading settings once it is published.

7. When you are happy with your settings, click Save Settings and Publish .
