HOW TO CREATE A SURVEY IN USAONLINE

USAonline offers you several kinds of polling and surveying tools. For one-question polls there
is Polls tool. If you want to create longer questionnaires, you might find it helpful to use Survey
Question Type in Tests and Quizzes.
1. After clicking Tests and Quizzes on the left side of the screen, create a new assessment
using Assessment Builder.
2. Choose Survey from the Add Question dropdown menu.

3. Type your question into the Question Text box (1), select the answer style (2) and click
Save. Add more questions to your survey by clicking the drop down menu next to Insert
New Question, as done in step 2.

4. After you are finished with creating questions for your questionnaire, configure your
settings.

5. Go to the Grading tab under Settings. Since USAonline automatically defaults the answer
point value to 0, you will need to change the Gradebook Options in the Settings to None.
6. If you want to keep students’ identities anonymous, select the Anonymous Grading Only
option. This option replaces students’ names with numbers, so you will not be able to view
what a specific student has submitted.
NOTE: You can update Grading settings for your published survey after students have
submitted their surveys to reveal the name for each submission. If you want to truly make
your survey anonymous, you should not change the Grading settings once it is published.

7. Once you are happy with your settings, click on Save Settings and Publish.
