Employment Paperwork: Complete at Human Resources,

□ Personnel Action Request (PAR)
□ W-4 (Employee’s Withholding Allowance)
□ I-9 (Employment Eligibility Verification)
□ Direct Deposit (www.byui.edu/financial-services)
□ Employee Dependents (Net ID Form)
□ Acknowledgement of Conditions of Employment
□ Employee Information Usage Agreement
□ Conflict of Interest Disclosure Statement
□ Work-Related Injury Reporting Procedure
New Employee Orientation: Includes a benefits orientation for
full-time employees; Human Resources, 226 Kimball
Network ID (Net ID): A Net ID allows access to the university
network (e-mail, calendars, policies, payroll notification,
personal account, etc.); Human Resources, 226 Kimball
Keys: Request supervisor approval for keys online at
www.byui.edu/university-operations/facilities-management.
Keys can be picked up at Locks & Keys, 82 Physical Plant, ext.
2551
Employee Picture: Schedule appointment for an official
employee picture with Jeff Smith, 350C McKay, ext. 7232
BYU-Idaho I-Card: Receive employee identification at the
I-Card Office. Request a PROX card. 130 Kimball, ext. 1931
Fire & Safety Training: Take required training at
http://www.byui.edu/university-operations/security-andsafety/fire-safety
Parking Permit: Receive a free faculty/staff permit at Parking
Services, 150H Kimball, ext. 3030
Privacy Training: Within 24 hours of starting, new employees
receive an email with instructions for completing the online

Accounting: Approve cash fund and signature authority;
Accounting Office, 130 Kimball, ext.1900
I-Series/AS400 or Faculty Web Access: Employees complete
the online Privacy Training; then supervisors receive and email
requesting approval for any expanded system access. (Direct

Keys: Approve email request generated by employee;
Locks & Keys, 82 Physical Plant, ext. 2551
Credit Cards or Purchasing Cards: Authorize cards through
Valerie Gutierrez, 290 Kimball, ext. 1901
Long Distance Calling Cards: Authorize cards through
Peggy Gibson, 130 Kimball, ext. 1921
SUPERVISOR ASSIGNMENTS
Dress & Grooming: Review appropriate dress standards for
employees and any specific requirements for the work area or for
special assignments.
Safety: Review the work area, including exits and safety/first aid
stations or supplies; provide safety training for work tasks.
Job Description: Review specific job responsibilities and
departmental practices and procedures.
Time & Attendance: Review leave policies, time and attendance
reporting practices, and availability of payroll notification. at
www.byui.edu/financial-services.
