You need this form if you are among the CSF seniors who need the spring semester’s grades to qualify for permanent membership (be a “CSF Sealbearer”). It is the sealbearer or “permanent member” distinction which will allow you to wear the CSF gold honor stole at graduation. There are two possible reasons that you are in this group:
1) You only have three qualifying semesters of CSF membership. This will make your fourth and final semester.
2) You may have four qualifying semesters of CSF membership, but none are based on senior grades.
The following is the procedure to qualify for CSF membership this last semester. Since you will graduate before we have a copy of your transcript verifying your qualifying grades, you must get your teachers to sign for the semester grade that you will earn in their class. These grades will be used in the same way that grades are traditionally used to qualify for
CSF (There must be no “D” or “F” in any course, and the points earned in column I, column II, and column III on the normal application form are subject to the same rules as were true for previous semesters). Please use this form to collect ALL your teachers’ signatures along with the grade that you will be receiving for the semester in their class.
