We occasionally photograph the children and assess their abilities for development studies. We need your
approval to include your child.

website.
I am/We are happy for the pre-school staff to administer first aid in the event of an accident.
I am/We are happy for my/our child to leave the pre-school premises to participate in organised outings, with
a ratio of one adult to two children.
I/We will supply a list of emergency contact names and telephone numbers.
I/we give permission for you to send foundation stage reports to my/our child’s first school or new setting.
In the interest of the child the Pre-school will share information with other relevant external agencies if
deemed necessary.
