provided during the first week of class. This includes all clinical/lab assignments, written
and presented projects (group/individual), job shadowing assignments, written reports,
medical competencies, participation in HOSA student chapter activities, maintaining a class
portfolio and medical notebook.

thus demonstrating professional behavior, attitude, and academic honesty.

Health Academy Confidentiality Contract
In order to properly prepare students for a career in the health care field, students are given the
opportunity to work with patients, residents, and clients in multiple healthcare settings. Students are
able to apply core skills learned in Medical Terminology/Anatomy and Physiology and Introduction to
Health Occupations.
Patients, residents, and clients who use the services provided by healthcare providers have the legal
right to confidentiality concerning all aspects of their care, including the fact that they used their
healthcare provider at all. In other words, anything concerning the healthcare used, where this it is
medical,
If a student violates that trust and breaks the law of confidentiality, it could result in the immediate
withdrawal of the student from any program and a withdrawal failure would be the grade received.
Participants in these programs are therefore required to sign waivers concerning confidentiality. Please
read and sign below prior to clinical experiences.
I____________________________will not discuss, recount, or in any form divulge any information
concerning patients, residents, or clients receiving healthcare in facilities were I am receiving instruction,
clinical experience, job shadowing, or anything pertaining to Career and Technical Education courses.
This includes family, friends, classmates, co-workers, or anyone else whom I have contact with. I
understand that I will be able to communicate with co-workers in the form of oral or written report or
during a case study as part of one of these named courses concerning certain aspects of the care of a
person using the health care facility. I understand that confidentiality applies also to any information,
which I may be told by anyone else. I will not use names of patients in any aspect of care. I will respect
the privacy and dignity of those entrusted to my care and learning.
