Dear Mr. Dumalag:
We hereby accept your bid for the above-stated Contract and therefore, award the Contract to you, as the
bidder with the Lowest Calculated Responsive Bid at a Total Contract Price equivalent to THREE
HUNDRED THIRTY FOUR THOUSAND EIGHT HUNDRED FIFTY ONE PESOS AND
TWENTY SIX CENTAVOS (P 334,851.26).
You are therefore required, within ten (10) days from your receipt of this Notice of Award, to submit to us
(a) this Notice with your signed “conforme” thereon, (b) the required Performance Security, (c) the
required Credit Line, and (d) the PERT/CPM diagram and Bar Chart with S-curve, (e) Construction
Methods, (f) Construction Safety and Health Program approved by the DOLE, all of which will form part
of the Contract Agreement in accordance with Bidding Documents. You are also required, within the said
ten (10) days, to formally sign the Contract Agreement. Your performance security shall remain valid
until issuance by this office of the final Certificate of Acceptance.
Your failure to comply with these requirements shall constitute a sufficient ground for the cancellation of
this award and the forfeiture of your Bid Security.

WHEREAS, the Department of Public Works and Highways, Iloilo 2nd District Engineering Office,
Balabag, Dumangas, Iloilo advertised the Invitation to Bid for CONTRACT ID NO. 13GG0079 CONSTRUCTION/REPAIR OF PAVEMENT, BRGY. SAN JOSE, POTOTAN, ILOILO in a
newspaper of general circulation and posted the same at the DPWH website, the G-EPS and a conspicuous
place at the premises of the Iloilo 2nd District Engineering Office, Balabag, Dumangas, Iloilo continuously
for 7 days starting from March 6, 2013;
WHEREAS, in response to the said Invitation, one (1) contractors submitted Letter of Intent and is
found eligible;
WHEREAS, the Approved Budget for the Contract (ABC) is in the amount of P 343,000.00;
WHEREAS, the BAC received one (1) bids for the contract and all passed the preliminary
examination;
WHEREAS, the bid proposals of the following bidders were found to be substantially complying;

NOW THEREFORE, We, the Members of the Bids and Awards Committee, hereby RESOLVE,
as it is hereby RESOLVED;

conducting the post-qualification of your firm/company involving that examination, validation and
verification of the eligibility, technical and financial requirements that you have submitted for the bidding
for this Contract, we are pleased to inform you that you have passed the post-qualification and therefore,
consider your bid as the Lowest Calculated Bid for the said Contract.
Upon the approval of this finding by the Head of the Procuring Entity, we will issue to you the Notice of
