Pre-Calculus Student Contract Mr. Simoncelli The Geneva High School community is proud to have measurable success found amongst its students and teachers. With that said, continued success requires certain measures of discipline and accountability. As young adults prepare to transition for life after GHS, it is critical to acknowledge college life as well as real-world demands. Being an active learner is vital to help you become successful. Please read and sign the following: 1. Electronic Device Policy (i.e. cell phones) In order to maximize active learning and be successful, we must minimize classroom distractions. Cell phones may serve valid educational needs in other contexts, but in this math classroom, they are a distraction. Thus, cell phones should be turned off at all times since they keep you (and others) from fully engaging in the lesson and discussion. I understand that consequences will be issued if cell phones are ON 2. Personal Accountability Policy Throughout the semester, students may need help with math problems. It is critical to seek the teacher’s help long before the actual test day. Furthermore, all students must take accountability for their work ethic and performance. Below is a checklist for all students to follow this year, and must be followed in order. I understand the course description as it pertains to my future goals I promise to take accountability for my work ethic and performance I have taken full, legible notes each day, checking over my work I have done my homework each day, checking given answers I have asked questions each day during homework discussion I have actively participated in several study groups with math classmates I have asked Mr. Simoncelli to come in for help before or after school After speaking with my parents, I have come in for help from Mr. Simoncelli: o (1 time the given chapter) o (2 times the given chapter) o (3 times the given chapter) o (4 times the given chapter) o (5 times the given chapter) I have pursued all of the above, and have discussed this with my parents A parent-teacher phone call with regard to my progress is necessary (parent-teacher contact is always encouraged at any point in time) Please sign if you have read through and discussed the syllabus & student contract. _____________________________ Student’s Signature _____________________________ Parent’s Signature
