CSE Style References Consulted Annotation Format
Name:
Paper / Project:___________________________
Directions: As you conduct your research and find usable sources, you will be completing an annotation for
each source on your own paper. Later, when you create your References Consulted page(s), you will use the

Sample Annotated References Consulted
References Consulted
[Anonymous]. 1998 October 20. Sale of Organs in the US from Executed Chinese Prisoners.
Directory of Persecuted Scientists, Health Professionals, and Engineers. Available from:
http://shr.aaas.org/dps/orgsale.htm. Accessed on: 2002 Jan 15. Medical ethics are the main idea of this
article. Various policies that conflict with the taking of organs in China are related to these policies. I took
one worldwide policy to use as an example. It shows that no matter what the culture there are certain rules
people must follow. Much of the information is from The New York Times and The New York Review. The
Directory of Persecuted Scientists, Health Professionals, and Engineers presents the information.
Berry P. 1999. Organ Donation: Can The US Do Better. British Medical Journal. 319(7222):1445-6. New
questions are presented about how organ donation waiting lists work. From personal experiences, the author
shares his opinions. I used two of the author’s ideas on organ donation. Both opinions were the opposite of
traditional views. Phil H. Berry Jr. is a past president of the Texas Medical Association. He has experience in
the subject, receiving an organ from a mother who died of an aneurysm.
Csillag C. 1996. Opting Out Of Organ Donation In Brazil. Lancet. 347(9003):754. The Brazilian senate passed
a bill that would make every citizen an organ donor. The bill suggested people opt out, rather than opt for
organ donation. I pointed out that others are going for the logical, not the normal. It helped to prove my
point of open-minded thinking. Claudi Csillag writes for Lancet, a scholarly medical journal.
Format Tips
1. Alphabeticize sources by last name
2. Don’t use first names
3. If no author, use [Anonymous]
4. Double space everything; use Times/Times New Roman—no Helvetica
5. Use 1” margins, BUT indent 2nd+ lines of a source 3 spaces
