I give permission for my child and guest to attend the Nottawasaga Pines Secondary School Prom
on Friday, June 5, 2015. I understand and agree that:

The school Code of Conduct and consequences will be in place at the Prom
I am responsible for the behaviour of my child and his/her guest
Students who violate the Code of Conduct will not be admitted to Prom and/or will be sent home
Persons who are believed to be bringing illegal substances to Prom are subject to a search by school
staff as per the Safe Schools Act.
I will be available by telephone that evening
I may have to pick up my child and guest if there are violations of the Code of Conduct
I am responsible for the transportation of my child and my child’s guest to a from the prom location.
The ticket price is non-refundable
