the Saint Lucie West Centennial High School Prom is a school sponsored event and my guest and I are
expected to follow the rules as stated in the SLWCHS Student Code of Conduct. I (we) may be
reprimanded by the SLWCHS administration for not following the stated rules and removal or
prevention from attending prom may also occur. I am further agreeing to the following stipulations by
initialing next to each item:

_____No casual or too revealing attire (we will check)
x Dresses cannot be more than 3 inches above the knees
x No excessive “dips” in the back, or extreme front/side slits
x NO EXCESSIVE CLEAVAGE
x SHIRTS MUST STAY ON DURING PROM

____Administration reserves the right to restrict a student from attending
Safety is always our number one priority. We want all students to have fun and get home safe, so we are asking
both student(s) and parents(s) to read and sign the prom promise. Pay careful attention to complete details in
the Student Handbook concerning the student Attendance and Conduct Policy. By signing below, both

Attendance and obligations must be cleared prior to ticket purchase for both Juniors and Seniors (no more than 13 absences during 2nd semester, starting 1/23/13).
Yes, your parent must sign even if you are 18 years old. NO REFUND if absent count is exceeded after ticket purchase! Guest cannot be older than 20 years old
