This is the Sponsor’s Fee application for the 2014 golf season (24 weeks) in the amount of $35.00.
Please make your checks payable to “CWGR Men’s Club”. Payment is due on or before April 2nd
2014, and can be turned into the Pro Shop or mailed to:

All team members and alternates must be members of the Men’s Club and be paid up by April 2nd,
2014. You must also sign up your team on the Wednesday League Sign-up sheet in the pro shop.
Limited to 36 teams. If there are more than 36 teams signed up, last year's teams and paying teams
have preference over CWCR employee teams.
