Job Description for Care Apprentice The main purpose of the job is to work as part of a team to promote and provide the highest standard of care within Fremantle’s services. Ensure completion of the apprenticeship programme and attain the BTEC apprenticeship qualification Line managed by Assistant Manager Key Responsibilities Provide respectful high-quality support and care for people who use Fremantle services in accordance with their individual Support Plans. Practically assist people who use services with tasks of daily living such as personal care and mobility, and end of life care, as directed in the Support Plan. Ensure that people who use services have companionship and social, emotional and leisure support in accordance with their wishes and the Support Plan Participate in developing the Support Plan with the person using the service and staff colleagues. Record essential information in people who use services’ daily reports, care and health monitoring charts, accident and incident reports, and service message systems clearly, accurately and immediately. Ensure that day-to-day tasks and routines are carried out to contribute to the smooth running of the service as a whole. Actively promote the service and Fremantle by clear, respectful and helpful communication with all potential customers including people who use services, relatives, health and social care professionals, other officials and contractors, visitors as well as Fremantle colleagues. Contribute to the success of the whole staff team by respecting colleagues and participating in team meetings and communications systems. Any other duties consistent with the business of Fremantle, the job, and hours of work as may reasonably be required.
