(It does not matter which address you type in. Each one takes you to the same website)

the required fields
Remember that your
Facility Number and
FIN are printed on the
mail out that you
received from DSHS
in September and/or
December 2010
Next, click the Submit
button

Write down your User ID
and Password. You will use
the same User ID and
Password to login to the
system next year.
