Company History:
WellPoint, Inc. was formed when WellPoint Health Networks Inc. and Anthem, Inc. merged in 2004. Anthem was
the outgrowth of two Indianapolis-based corporations formed in 1944 and 1946 as mutual insurance companies,
commonly known as Blue Cross of Indiana and Blue Shield of Indiana. WellPoint Health Networks Inc. was formed
in 1996 through the merger of WellPoint and Blue Cross of California into a single stockholder-owned company.

Organizational Structure
Lumenos is a wholly owned operating division of WellPoint, was acquired in the summer of 2005. Lumenos offers
consumers Health Savings Account plans (HSA) and Health Reimbursement Account plans (HRA), information,
services and incentives to become better informed about their health and engage in health-promoting behaviors.
HealthLink provides network rental for workers’ compensation and health benefits programs in Missouri, Arkansas,
Illinois, Indiana, Iowa, Kentucky and West Virginia.
