This form is to help you in deciding if you need to complete a tax return, this section only applies to
individuals who have never completed Tax Returns before.
If you have at any time in the past completed Tax Returns you should contact HMRC on 0845 300 0627
for advice.
If you have received a tax return or a notice to complete one you will need to complete the tax
return regardless of your circumstances. Skip this section and complete the rest of the form.
If you wish to take advantage of the Tax Return service offered, please check the list below entitled ‘Do I
need to complete a tax return’ to see if a tax return needs to be completed on your behalf.
Do I need to complete a tax return?

What you need to do
If you do need to complete a tax return you must advise your tax office that you need to complete a tax
return on the basis of any of the above statements you have answered ‘yes’ to. HMRC can be contacted
on 0845 300 0627
They will then set a Self Assessment record up for you, you should ask them to send you your UTR
number as this will be needed to submit your tax return to HM Revenue & Customs.
If you wish RSM Tenon to complete a tax return for you, please use the 2012 tax return information pack
to provide your income and personal details.
What’s the cost
The cost of completion of the tax return is charged at the discounted rate of £90.00 (incl VAT).
