Senior Project Parent/Guardian Overview Meeting-6:00pm in the
PHS Cafeteria.
25 points of extra credit.
Checkpoint #1: Proposal Form- Mentor Information and fieldwork
proposal must be included along with mentor signature. An
annotated article, from a reliable source, must also be attached to
the proposal.
100 points earned once proposal has been APPROVED.
Checkpoint #2: Senior Project forms with signatures due:
Senior Project Contact, Liability Release Form, Plagiarism &
Honesty Form and completed Mentor Commitment Form.

Checkpoint #3: Fieldwork-Proof of first 5 Hours: 1 picture with
student and mentor, 1 picture of student shopping, gathering
items or drafting, and 3 additional pictures demonstrating
proof of project outlined in the student’s proposal. 5 pictures
in total, and student must be in all pictures. For each picture,
students must write a 5-7 sentence paragraph explaining what
is occurring in the picture along with reflecting on their
struggles and/or successes in working on this specific portion
of the project.; Fieldwork timeline signed by Mentor (reflects
a min. of 5 hours).

Checkpoint #4: Senior Project Teacher Read-All paper requirements
must be met: Title Page, Outline, pages 1-7/11 pages & works cited.

Checkpoint #5: Fieldwork- Proof of 10 Hours: Another picture
with the mentor (1) plus 4 pictures demonstrating student
working on the proof of project. At this point, you need a total
of 10 pictures (5 pictures from CP#3). Student must be in the
pictures and must be completing a portion of the proof of
project. For each picture, students must write a 5-7 sentence
paragraph explaining what is occurring in the picture along
with reflecting on their struggles and/or successes in working
on this specific portion of their project. Fieldwork timeline
signed by Mentor (reflects a min. of 10 hours). 100 points
Checkpoint #6: Final Paper-Staff Read. All paper requirements
must be met: Title Page, Outline, pages 1-7 pages & works
cited.

Checkpoint #7: Fieldwork-Proof of 15 plus Hours: A final picture with
the mentor, 4 pictures demonstrating the near completion of the
proof of project. At this point, you need a total of 15 pictures (5
pictures from CP#3; 5 pictures from CP#5). The student must in the
pictures and must be near completing the proof of project. For each
picture, students must write a 5-7 sentence paragraph explaining
what is occurring in the picture along with reflecting on their
struggles and/or successes in working on this specific portion of their
project. ALL additional hours & proof of hours due. 100 points
Checkpoint #8: Completed Portfolio-1 Bound Copy with ALL required
documents. 100 points
Checkpoint #9: Practice Presentation-Must practice present for SP
teacher in order to receive an SP Board Appointment. Practice
presenters must dress in professional attire, must have speech
cards, Power Point/Prezzi must be completed and physical project
must be completed. Students MUST reach the 7 minutes time limit
and not go over 11 minutes.
