Please email the completed registration, group budget document, and event outline (if necessary) to volunteer@jhu.edu by Tuesday, September 8, 2015 at noon If you wish to return as a CSC student group, you must submit your group re-registration form by the deadline. If you have questions about any of the items below, please provide as much information as possible and discuss it with your advisor during your re-registration meeting. You will have the opportunity to revise this form after your conversation with your advisor. NOTE: Student groups forming at service sites that are new to CSC’s agency network will not be able to serve or receive group funds until their agency signs a Memorandum of Understanding. Statement of Need (50-200 words) Please describe in more detail the community-identified need your group addresses. How have you examined and evaluated this need? Who is your target audience(s)? What gap in services and/or programming are you filling in support of your community partner and, ultimately, your target audience? What expectations do you have of potential members (i.e. number of hours required, meeting attendance requirements)? 5. Description of Community Partner (50-500 words) Please describe the mission and goals of your partnering organization(s). Please name your contact at your partner organization and describe the relationship and communications you have had with him or her. What plans have you made with your community partner regarding continuing to serve during the 2015-2016 academic year? 6. Goals for your Student Organization in 2015-2016. Please list at least three key SMART (Specific Measurable Achievable Relevant Time-bound) goals for your organization, including both immediate and long-term goals. What is the overall purpose of the program? What are the aims of the program in terms of providing service to community partners, supporting student volunteers, and building relationships between campus and community?. EXAMPLE S – We will recruit new volunteers for our program. M – We are looking for 10 new volunteers to tutor twice a week. A – We will begin recruiting new volunteers at SOHOP in the spring as well as the fall activities fair; we will do an orientation with all new volunteers so they have a better understanding of what is going on and pair them with a returning volunteer who will check in with them to help increase retention. R – New volunteers will help us to lower the tutor/tutee ration for our program. T – We will recruit new volunteers by October 1, and start the volunteer mentoring program by November 1. Tips for success: Please be as specific as possible. “Raising awareness about homelessness” is difficult to measure. Consider including a goal related to strengthening your organization internally to pave the way for a sustainable organization. Your goals must address community-identified needs. Please be sure to discuss your proposed goals and objectives with your community partners to make sure they are focused on the most pressing challenges the community is looking to address. Focus on program quality, not program quantity – expanding the number of service sites is often not the best way to grow the organization. Measuring the success of your goals is very important – how might you evaluate whether you are successfully achieving what you are setting out to do? Methods of evaluating goals include preand post-tests; surveys; and taking attendance. We will send an email later this summer with instructions on how to set up your advising appointment, which will take place between Thursday, September 10 and Thursday, September 24. Please mark your calendar for the CSC Leadership Conference, which will take place on Saturday, September 19. All returning groups must send at least two delegates to this conference.
