Project Title: Request for Proposal for Vital Signs Monitoring RFP Reference No: MHHISDRFP16219 RFP Roll-Out Date: 12 February 2016 RFP Closing Date: 18 March 2016, 17:00 hrs (SG Time) 1. Objective of Project MOH Holdings Pte Ltd (MOHH) is pleased to invite proposals to provide a cost-effective Vital Signs Monitoring (VSM) solution and telehealth services to facilitate a large scale and coordinated community VSM pilot with acute hospitals to mitigate increasing demand for acute hospital resources. The common VSM solution should also set the stage to propel VSM service forward as more care models are introduced to tap on VSM. The key diseases/conditions that we are focusing on for this project are: a) Congestive Heart Failure (HF) b) Diabetes Mellitus (DM) c) Chronic Obstructive Pulmonary Disease (COPD) d) Acute Myocardial Infarction (AMI) e) Stroke The key vital signs or parameters to be supported in this project are: a) Blood Pressure b) Blood Glucose c) Weight d) Pulse e) Temperature f) SPO2 g) Steps/Flight of Steps/Calories/Distance Travelled The general components we are sourcing include: a) Provider and Patient Interface systems for providers and patients respectively to manage vital signs data and VSM programmes. b) Provision, management and maintenance support of monitoring devices, particularly devices that support auto-transmission of vital signs and parameters into the Provider and Patient Interface systems. Vendors must be able to provide devices for all the key vitals and parameters listed above. c) Professional services in the area of providing clinical programme specific workflow development, resource planning and staff training to the institutions. d) Proven large scale, concurrent project implementation experience over multiple programmes, stakeholders and healthcare institutions within a short period of time (e.g. delivering 4 or 5 VSM programmes over 3 months). 2. Participating Procedures The Request for Proposal (RFP) process shall be as follows: 1) Interested Vendor shall first read the Confidentiality Undertakings Letter (“Letter”) Page 1 of 3 and Annexure 1 attached in Annex A to this Notice. The vendor shall also read the eligibility criteria and fill up the Eligibility Information Form in Annex B to this Notice. The vendor must meet the following eligibility criteria: - No existing lawsuit or claim or bankruptcy; - No existing corruption charges; - No conflict of interest due to significant capital investment; - Already rolled out VSM trials/programs/pilots to more than 50 patients in support of healthcare providers’ clinical programmes; - Has distribution rights to loan or sell medical devices to automatically capture and transmit data captured for the following vital signs/physical parameters (Blood Pressure, Blood Glucose, Weight, Heart Rate/Pulse, SPO2, Steps/Flights/Calories/Distance) 2) If the vendor is agreeable to be bound by the terms of this Letter and Annexure 1 and fully meets the eligibility criteria, the vendor may proceed to request for the electronic RFP documents by sending an email to procurement@mohh.com.sg. In the email, please provide the following: − − − − company name; company registration number; business address and the name, designation, contact number and email address of your company representative who will be the primary contact for all communications with MOHH pertaining to the RFP (“Representative”), with the Subject line as “Response to MHHISDRFP16219- Vital Signs Monitoring” − completed Eligibility Information Form (Annex B) and all supporting documents in an attachment 3) Closing date for the submission of Eligibility Information Form will be on 26 February 2016. 4) MOHH will perform the eligibility assessment of the requests. Only vendors who have been assessed by MOHH to fulfil the eligibility requirements will be released the electronic RFP documents. 5) Instructions relating to your submission of Proposal will be stipulated in the RFP documentation. Only Proposals submitted in compliance with these instructions will be considered. Please take note to sign and deliver one (1) original copy of the Letter and Annexure 1 together with your RFP proposals. 6) A vendor briefing will be held on 3 March 2016 for eligible vendors. Invitation will be sent to your representative via email.
