doctors, nurses & paramedics.

We hope this booklet will provide the information necessary
for you. In case of any query, please call our Patient Welfare
Officer at extension 6016 or Public Relation Officer at
extension 6018.
Your Arrival at SDM Hospital
Your doctor admits you to SDMH. Your room assignment
is determined by your doctor's orders, the type of medical
care you are to receive, and the availability of rooms. Your
admission to the hospital will be processed without regard
to race, creed, color, or national origin.

health care at the most affordable rates to all sections of
society, irrespective of cast, creed or color. Towards making
this possible, we pledge to dedicate all our efforts and all our
energies.
Patient's Belongings
Hospital is a public place. Kindly take care of your belongings
and valuables such as mobile phones, ornaments, cash etc.
Kindly keep them safely in the locker provided to you.
Payment Conditions
1. Hospital Charges are not negotiable.
2. Hospital Tariff is available at the reception and with
Patient Welfare Officer.
3. In case the patient has to undergo an operative
procedure, patient/attendant has to deposit the full
amount before the operation or at the time of
admission only.

4. The Hospital charges for Deluxe & Super Deluxe rooms will
be double of General ward charges. However for Private
room, these will be 1.5 times the charges for General ward.
5. For all patients transferred from General ward to rooms
and vice- versa the charges (for investigation, procedure,
operations and visits etc.) would be applicable as for the
room, availed from the date of admission.

6. Nevertheless from 11 day onwards only, the charges
(Investigation and other procedures, visits) shall be
charged as applicable for General ward / Room.

In case the patient is in ICU/OT, the room can be
retained by paying the room charges.
8.
Provisional bills will be presented every 7 days for all
wards / rooms and every 3 days for ICU patients and it
should be cleared by next morning.
9.
CHANGE OF ROOM- The Management reserves the
right to change the room allotted to any patient at any time.
10. Please get receipt of all payments made.
11. For payment only cash, Credit Cards, Demand Draft,
pay order (In favor of SDMH) are accepted. Cheques
are not accepted.
12. Refunds above Rs 50,000/- would be made by A/C
Payee cheque only.

13. During banking hours, payment of final bill has to be
paid in Union Bank of India-SDMH Branch.
14. Empanelled with all Major TPA's for cashless treatment
(Medicines, CT Scan & MRI are excluded).
15. 10% Rebate for State Govt. employees (appointed
before 1st Jan 2004) is available on bed charges,
pathological examination, OT charges, & Radiological
Examination.
16. 24 hrs Ambulance services are available on payment
basis only.
Visiting Hours
•
Visiting hours are from 5 pm to 8 pm.
•
One permanent pass will be issued per room/ bed
allocated at the time of admission. If suggested by
doctor one visitor pass will be issued on payment basis
only.
•
In order to allow our patients to rest and recuperate, it is
our policy to limit visitors to only one (1) per patient.
•
For reason of Infection control, Children under the age
of 12 & flowers are not allowed in any patient areas.

Food & Beverages
The chief objective of dietetics department is to serve
healthy, nutritious and disease specific delicacies to the
patient as per the recommendation of doctors and dietician.
Attendant diet can be provided on payment.
Patient Rights and Responsibilities
All patients have right to:1.

Refuse treatment and right to have a second opinion.

Know the identity of individuals providing service.

Information on treatment options available.

Informed consent.

Know hospital rules & charge schedules, any
concessions.

initiated.
12. Access to medical records.
13. Special preferences, spiritual and cultural needs.
14. Protection from physical abuse or neglect.
Patient's Responsibilities to the Hospital:
1. Provide complete and accurate information.
2. Abide by all hospital rules and regulations.
3. To pay for services bill in a timely manner as per the
hospital rules.
4. To accept, where applicable, adaptations to the
environment to ensure a safe and secure stay in hospital.
5. To accept the measures taken by the hospital to ensure
personal privacy and confidentiality of medical records.
6. To attend follow up appointment as requested.
7. Not to take any medications without the knowledge of
doctor and health care professionals.
8. To provide correct and truthful history.
9. To understand the charter of rights and seek clarification if any.

Discharge
Check out time is 12 noon. When you are being discharged
from the hospital, your doctor and nurse, dietician, or
pharmacist will explain the treatment plan which you will
follow at home.
Feedback
For Evaluation of hospital services, you are requested to fill
