Machine Shop Safety Agreement

the Lathe, Drill Press, and Grinding Wheel.
______ I have reviewed all the safety videos on the Machine Shop Safety webpage.
______ I have completed the “Hand and Power tool safety” “Machine Guarding” and
“Electrical Safety” training on the Machine Shop Safety webpage.
______ I understand, and will adhere to the Lock out/Tag out procedures.
______ I will clean the shop directly after I use it (vacuum, throw out trash, etc.)
______ I will not store my materials in the shop.
______ I will wear the appropriate PPE when working in the shop, as per the SOPs.
______ I have read and will adhere to the injury protocol.
______ I have read and understand the Job Safety Analysis for Shop Workers.
______ I understand that if I violate this agreement, my access to the Shop may be
terminated.
