Part 1: Advertising and Promotional Release
I hearby consent to the reproduction and/or use of photographs, DVDs, and film and audio recordings of
myself (or my child) for advertising and promotional purposes only by Bear Stage Production of
Buchanan High School CUSD or its affiliates.

Part 2: Conflict Regulations (See Calendar)
I have listed every rehearsal that I will miss during the rehearsal period. I understand that any absences
from a scheduled rehearsal, including the rehearsals I have listed above, can result in my exclusion from
portions of the show. That includes absences due to illness, as well as commitments to school, church,
family, etc. I understand that this is not offered as punishment. With a very complicated production &
short rehearsal periods, each rehearsal must progress (regardless of my absence) to each opening night
with a great show. There will be no time to “save a place” for anyone. I understand that missing a
performance is not permitted without permission from the director prior to casting.
