Instructions For Filling Out Your
Blue Card
Your Scoutmaster/Unit Leader should have
blue cards or can help you get one. In order to
expedite the signing of your BSA Merit Badge
Blue Card, it must be COMPLETELY filled out
per the example below, or your Blue Card will
not be signed by the Merit Badge Counselor.
“Be Prepared” and do this BEFORE you arrive
at Knott's Berry Farm. Please Be Legible, so
write Carefully! Make sure you have entered
every piece of information on the following
example including the requirement table and
Merit Badge Counselor’s contact information.
