As the intake doctor, you have spent the past several hours listening to Holden, uncovering numerous elements of his life that lead him to the hospital. You must present your findings to the admission director, offering a diagnosis and a recommended course of treatment to best help Holden recover. Step 1 – Intake Chart – Complete In Class Thursday, December 4th Working with your Lit Group, gather information from the novel to identify “facts” about Holden’s background, symptoms and what recent traumas he may have experienced. Complete the first side of the Patient Intake Chart included in this packet. Step 2 – What’s the Diagnosis? – In Class - Meet in Computer Lab rm 621 – 12/5/08 Taking the information that you gathered with your Lit Group, you will research numerous mental conditions/illnesses that might fit Holden’s profile. Using the electronic databases and mental health websites, gather information that will help identify what condition best fits Holden’s symptoms. You will need to quote or paraphrase your sources as part of your diagnosis. Be sure you have printed or saved the web page information and completed a research card for each source before you leave today! Although you may not have finalized your diagnosis by the end of the block, you will need to present the attached Diagnosis Overview Sheet by Monday’s class (that’s December 8th) Step 3 – Convincing the Admission Director (aka – writing the essay!) Although you are a “medical profession” (ha!) you do not have the authority to admit Holden into the hospital. Instead, you must present your findings to the Admissions Director who must be persuaded to accept your diagnosis and suggested course of treatment. Holden’s mental health is in your hands! Using the essay packet and instructions, you will write a persuasive report that will include the following: 1. Background – So that all who read the report will be able to know the necessary details that brought him to you in the first place, include the key information that will support your diagnosis. 2. Analysis – Using quotes from the text, identify the issues that Holden is facing. This may include people, events, internal thoughts or external elements that have had an impact on Holden’s mental state. 3. Diagnosis – Taking the information you offered in your analysis, and the research material your gathered, make a diagnosis of Holden condition. 4. Prescription – Based on your diagnosis, what treatment does Holden need to follow to restore his mental health? This should include both what he should do while in the hospital and what out-patient care he should also follow. 5. Prognosis – Based on your diagnosis and prescribed care, what is your prediction of Holden’s long term health.
