Hypertension Nurse Specialist Croí is a registered Irish Heart & Stroke Charity based in Galway. Its mission is to prevent cardiovascular disease, save lives and promote recovery and wellbeing. Over the past decade, Croí have developed an expertise in cardiovascular disease prevention, early detection and intervention which has achieved national recognition through a variety of healthcare and innovation awards. With support from the Medtronic Foundation, we are embarking on an exciting new initiative in the detection and management of hypertension in the West of Ireland. We are seeking to recruit a ‘Hypertension Nurse Specialist’ to set up and co-ordinate this new programme which aims to screen for hypertension in the community and promote the integration of care between primary care and hospital. This is initially a 2-year project which will require the nurse lead to develop and deliver a public education and awareness campaign; a community hypertension screening programme; work with primary care to identify difficult to manage and resistant hypertensive patients and refer/liaise with specialist hospital services. Principle Duties and Responsibilities Support the research agenda of the ‘West of Ireland Hypertension Programme’ Co-ordinate and deliver health promotion information sessions to raise awareness of hypertension in the community Conduct community hypertension screenings Provide expert nursing advice and referral to primary care. Work with primary care physicians in identifying and referring resistant hypertensive patients to specialist clinics Operate as a member of the Croí Community Cardiovascular Health Team in delivering screening and early intervention and facilitating behaviour change using motivational interviewing, brief intervention techniques and goal setting Participate in research and clinical governance audit Skills and Competencies The post holder will be required to demonstrate: A proficient knowledge in the prevention and management of cardiovascular disease, with a particular emphasis on hypertension An understanding of research methods Effective communication and interpersonal skills The ability to work autonomously and as part of a team to achieve key performance outputs Initiative and motivation in service delivery Confidence, assertiveness and flexibility Excellent IT skills to include use of Email, Microsoft Word, PowerPoint and Database management Excellent organisational skills with an aptitude for effective time management, problem solving, project planning, delivery and execution Advanced clinical reasoning A commitment to quality measurable through audit and research Requirements The post holder must be suitably qualified as follows: Be registered with An Bord Altranais Have at least 4 years relevant post registration experience, one of which must be in the area of cardiology/stroke/research Have the clinical, managerial and administrative capacity to fulfil the functions of the role Experience in the area of Cardiology, Stroke, or Research is essential. A postgraduate qualification in any of the following would be an advantage: Cardiac Nursing Practice Nursing Cardiac Rehabilitation Health Research Diabetes Health Promotion Remuneration This appointment will be made as a fixed term contract for 2 years.
