Prevention Nurse Co-ordinator Croí is a registered Irish heart and stroke charity, committed to excellence in cardiovascular disease prevention, early detection, rehabilitation, education and research. It’s record of innovation in promoting cardiovascular health and well-being, has achieved recognition through a variety of health care and innovation awards. Croí has recently opened a dedicated ‘Heart and Stroke Centre’, being the first of its kind in Europe. This centre provides a range of cardiovascular health and lifestyle programmes, and is committed to developing substantial education and research activity. Primary Role To co-ordinate and deliver a range of cardiovascular disease prevention & education initiatives and to co-ordinate collaborative clinical research in the areas of stroke, diabetes, heart disease and obesity. Principle Duties and Responsibilities Co-ordinate, plan and implement newly developed Croí Community Cardiovascular Health programmes which include; cardiovascular risk factor screening, lifestyle and behaviour change; public awareness campaigns and general cardiovascular health promotion; Co-ordinate and manage clinical research and audit; Deliver to an established protocol, full clinical assessment, management and evaluation of individuals and families enrolled on Croi Prevention programmes, providing expert nursing advice, support and counselling; Co-ordinate and deliver health promotion information sessions to members of the general public; Facilitate behaviour change using motivational interviewing, brief intervention techniques and goal setting; Be responsible for your own continuing professional development; Skills and Competencies The post holder will be required to demonstrate: A proficient knowledge of current guidelines for prevention and management of cardiovascular disease; An understanding of clinical research methods; Effective communication and interpersonal skills; The ability to work autonomously, as well as part of multi disciplinary team to achieve key performance outputs; Motivation and initiative and in service delivery; Confidence, assertiveness and flexibility; Excellent IT skills to include use of Email, Microsoft Word, PowerPoint and Excel; Excellent organisational skills with an aptitude for effective time management, problem solving, project planning, delivery and execution; Advanced clinical reasoning; A commitment to quality measurable through audit and research. Requirements The post holder must be suitably qualified as follows: Be registered with An Bord Altranais; Have at least 4 years relevant post registration experience, one of which must be in the area of cardiovascular disease prevention/health promotion/clinical research; Have the clinical, managerial and administrative capacity to fulfil the functions of the role; Demonstrate evidence of a commitment to continuing professional development. Experience in the area of Cardiac Rehabilitation, Health Promotion or Clinical Research as well as a postgraduate qualification in any of the following is desirable: Cardiac Nursing Cardiac Rehabilitation Practice Nursing Clinical Research Diabetes Health Promotion Health Psychology Remuneration This appointment will be made as a fixed term contract of 12 months.
