XXXX is a happy and social XXXX year old girl. She has spastic quadriplegia post herpes
encephalitis occurring in the infantile period. She has multiple medical issues including severe global
development delay, seizure disorder, precocious puberty, gastrotomy tube fed with a history of
aspiration pneumonias and reflux, chronic constipation, and scoliosis. XXXX experiences recurring
respiratory illnesses. She has also experienced progressive spasticity in her limbs resulting in
decreasing performance in both her legs and arms. XXXX was most recently referred to Sunny Hill
Health Centre for assessment, which was completed June 20, 2007. XXXX requires daily chest
physiotherapy exercises to improve her respiratory function. She is taking Baclofen for her
increasing spasticity to prevent further contracture deformities in her arms and legs. She requires full
assistance for daily living. She is confined to a wheelchair and is fed through a g-tube. XXXX has
some hearing loss and wears corrective lenses for her vision. She communicates through eye gaze,
facial expressions, reaching, and some vocalizations.
