The Alliance Leadership and Innovation Council 2015 Council Member Nomination Form Overview: The Organ Donation and Transplantation Alliance (The Alliance) is seeking nominations of individuals to serve on the newly created Leadership and Innovation Council. This Council will be comprised of not only organ donation and transplantation industry leaders but also innovative leaders from other industries including, but not limited to, higher education, healthcare, technology and business. Alliance mission: The Alliance is the recognized leader within the organ donation and transplantation community dedicated to providing engaged learning, innovation and collaborative leadership for future advancements in organ donation and transplantation. Purpose: The Leadership and Innovation Council will facilitate leadership networking and innovative idea exchange, testing and implementation within the organ donation and transplantation community in an effort to save and heal more lives. Council Principles: Lead and influence the organ donation and transplantation community Collaborate and engage with the next generation of donation and transplantation leaders to share knowledge and ideas and transform ideas into action Engage the donation and transplantation community to encourage broader, innovative thinking for the purposes of increasing the number of organ donors and patients transplanted in the nation Council Member Commitment: Council members will serve a two year term of appointment and may be reappointed for one additional two year term. Members will participate in four quarterly conference calls each year and one in-person meeting. Members will be responsible for covering the travel costs associated with attending any in-person meetings. If a member or their organization is unable to provide travel support, the member may apply for a travel scholarship to help offset travel expenses for attending the in-person meeting. Council Member Attributes: The Council will be comprised of leaders, change agents and innovative thinkers within their respective industry. To the extent possible, members’ backgrounds will include senior leaders, clinical operations and performance improvement professionals who demonstrate the ability to effectively lead change. Every effort will be made to represent diversity and a variety of geographic locations in the United States. Members of this Council must possess specific attributes in order to best carry out the mission and purpose of the Council. Members must be experienced leaders or come recommended by a leader, and all must have a proven track record of innovation and being an effective change 1 agent. This must be demonstrated through submitting examples of their accomplishments in how they impacted change within their organization and/or industry. Required attributes: 1. Innovative thinker 2.
