DISCLOSURE OF PROTECTED HEALTH INFORMATION FOR
CADAVERIC ORGAN, EYE, OR TISSUE DONATION
SINDECUSE HEALTH CENTER HIPAA POLICY
WESTERN MICHIGAN UNIVERSITY
POLICY:
The Sindecuse Health Center (SHC) may disclose Protected Health Information
(PHI) to organ procurement organizations or other entities engaged in the
procurement, banking, or transplantation of cadaveric organs, eyes, or tissue for
the purpose of facilitating organ, eye, or tissue donation and transplantation.
The SHC may make a disclosure under this policy without obtaining consent,
authorization, or giving the individual the opportunity to agree or object.
PROCESS:
1. The SHC will follow its existing policies related to notifying organ procurement agencies,
making family members aware of the option of organ or tissue donation and of their option to
decline, and obtaining consent for donation in accordance with federal and state law.
2. The SHC Director will follow appropriate policies and procedures for verifying the identity
and authority of individuals requesting PHI in connection with a donation and disclose PHI
only to persons whose identity and authority have been verified.
