Adaptive Special Education KSDE Assessment 12 TRANSITION PLANNING ASSIGNMENT Each student will develop a transition plan based on a current student that is on your caseload or using one of the scenarios provided by the instructor. The transition plan must include the following information: (a) anticipated post secondary goals for the student, (b) instruction services needed, (c) community experiences needed, (d) development of employment and other post-secondary adult living objectives, (e) acquisition of daily living skills, and (f) functional vocational assessments.
