POST: CONSULTANT GENERAL PHYSICIAN WITH AN INTEREST IN
GASTROENTEROLOGY
BASE: ROYAL ALEXANDRA HOSPITAL
We are looking to appoint three Consultants to join our team of seven
Consultant Physicians with an interest in Gastroenterology based at the Royal
Alexandra Hospital in Paisley.
Post holders will be required to participate in acute medical receiving and
provide care for inpatients, outpatients and daycases. The endoscopy
departments provide a full range of diagnostic and therapeutic endoscopy.
Sub speciality services within this service include hepatology, inflammatory
bowel disease, clinic nutrition and upper gastrointestinal disease and the
successful candidates will have the opportunity to develop a relevant sub
speciality interest.
The Royal Alexandra Hospital (RAH) is a 968 bedded modern district general
hospital in Paisley serving the population of approximately 205,000. The RAH
is one of the largest and busy non teaching hospital in Scotland. All major
specialties are represented within the hospital. The hospital has a
progressive agenda in relation to clinical standards, managed clinical
networks and patient focused service re-design. There are close clinical links
with the Community Health Partnership, the other acute hospitals within NHS
Greater Glasgow and Clyde.
Paisley is situated approximately 15 minutes from Glasgow, and located only
5 minutes from the M8 motorway and Glasgow International Airport, the RAH
provides an ideal centralised location.
Royal Alexandra Hospital has a team of twenty two Consultant Physicians,
five Specialist Registrars, eight CMT/GPST Doctors, six Foundation Year 2
and seventeen Foundation Year 1 Doctors.
The post holder will be required to maintain and develop the gastroenterology
service and work with colleagues to progress service delivery for patients
across Clyde.
MRCP UK or equivalent qualification is essential and you must have full
registration with the GMC and a licence to practice. Those trained in the UK
should have evidence of higher Specialist Training leading to a CCT in
Gastroenterology and General Acute Medicine or eligibility for specialist
registration (CESR) or be within six months of confirmed entry at the date of

This post of Consultant Physician with an interest in Gastroenterology will be
based at the Royal Alexandra Hospital (RAH), Paisley, five miles from the
bustling West end of Glasgow. The successful candidate will integrate into a
team comprising seven Consultants to develop services within
gastroenterology across Clyde. This will offer a real opportunity for the
successful candidate to be involved in the development of a successful
modern consultant led service for a population of around 400,000.
1.General Information
The Royal Alexandra Hospital (RAH) is situated in Paisley and provides
acute health care services to Renfrew District (pop 205,000). The
Hospital is a 968 bed modern District General Hospital (DGH) which
opened in 1986 and is one of the largest and busiest non-teaching
DGHs in Scotland. In addition the hospital is currently the base for
provision of surgical, orthopaedic, ophthalmology, paediatric and the
majority of maternity services to the population north of the Clyde
served by the Vale of Leven Hospital.
The hospital provides a wide range of District General Hospital
specialties with excellent support facilities. The hospital has a first class
modern radiology department and services. All departments participate
in undergraduate teaching and there is an active postgraduate
educational programme. The hospital enjoys an enviable reputation for
undergraduate teaching and is highly popular with training grades.
There is a good medical library service.
Inverclyde Royal Hospital is a modern district general hospital in
Greenock, and serves the Inverclyde District (Greenock, Gourock and
Port Glasgow), Largs, Fairlie, Skelmorlie, Wemyss Bay, Bute and the
Cowal Peninsula, a total population of approximately 120,000
comprising both urban and rural populations. The hospital was opened
in 1979 and currently has 342 beds.
All major specialties are represented within the hospital, with a 90bedded Geriatric Hospital and 8 bed Physically Disabled Rehabilitation
Unit.. There is a dedicated hospital wide academic programme
supported by a strong departmental educational commitment. The
hospital is well provided with a dedicated education center and IT
support. The hospital has an excellent reputation in supporting and
nurturing its junior staff and has an excellent reputation for post
graduate training.
The Vale of Leven Hospital (VOL) is situated in Alexandria, an area of
outstanding natural beauty and serving a population in excess of
88,000 largely from Dumbarton, Alexandria and Helensburgh. Services
at the Vale of Leven Hospital include unscheduled medical care led by
the Consultant Physician team based at the Royal Alexandra Hospital
and supported by GP and GP trainees. In addition there is inpatient
medicine for the elderly beds, short stay surgical beds together with
day surgery and a full range of diagnostic and support services.
2.The Medical Unit
The Medical Unit at the Royal Alexandra Hospital has 209 beds. This
includes an 18 bedded Coronary Care / Chest Pain Unit, a 30 bedded
medical receiving ward and Medical Assessment Unit.. The medical
unit provides an acute service in general medicine with clinics in
general medicine, diabetes and endocrinology, gastroenterology,
cardiology, asthma, respiratory medicine and haematology.
The Medical Unit at Inverclyde Royal consists of an Acute Medical
Receiving Unit and three general medical wards totaling 116 beds and
an 11 bedded Coronary Care Unit. The Medical Unit provides an acute
service in general medicine with clinics in general medicine, diabetes,
endocrinology, cardiology, asthma and respiratory medicine in addition
to gastroenterology.
The Vale of Leven Hospital medical unit includes a Medical
Assessment Unit, 12 bed receiving unit with provision for monitored
beds and a further 27 general medical beds.

There are Specialist Nurses in Diabetes, Respiratory Medicine, Cardiac
Rehabilitation, Cardiac Failure, Haemato-Oncology, D.V.T and Chest
Pain management and Inflammatory Bowel Disease across Clyde.
6.
Gastroenterology Services
6.1
There are thirty nine in patient beds at Royal Alexandra Hospital
provided across wards 27 and 14 for Gatroenterology.
6.2
Staffing for Gastroenterology services in Clyde will be seven
Consultant Physicians.
6.3
Nurse specialists in Inflammatory Bowel Disease (3), Hepatitis C (2),
Liver Nurse Specialist (1) and Alcohol Liaison are already in post. An
anorectal and oesophageal manometry service is provided by Surgical
Nurse Specialists.
6.4
Daycase and outpatient facilities include: Two endoscopy rooms in the
Day Surgery unit at RAH, two rooms available at IRH and one room
available at VOL. ERCP is carried out by two Physicians and one
Surgeon.. Outpatient services are provided on the three Hospital sites.
6.5
There is an Alcohol Liaison Service.
7. Acute Medical Services
The team of Consultant Physicians at the Royal Alexandra
Hospital have responsibility for acute medical services at both the
Royal Alexandra Hospital and the Vale of Leven Hospital.
7.1
Acute Receiving Ward
GP referred medical patients are triaged from the Emergency
Department to the Medical Assessment Unit at Royal Alexandra
Hospital Monday to Friday 10am to 10pm. All patients are reviewed by
a senior doctor of which just under 50% are discharged home. Patients
requiring admission can be admitted to the relevant specialty bed direct
from MAU.
Acute medical admissions are admitted to the Acute Medical Unit (ward
2) at the RAH, which is staffed by one of the consultants on a rotating
‘Physician of the Week’ basis Monday morning to Friday lunchtime and
an Acute Care Physician. Patients are then triaged to the appropriate
specialty wards if not for early discharge.
Acute medical admissions at the VOL are admitted to ward 3, staffed
by a third consultant on a rotating ‘Physician of the Week’ basis
Monday morning through to Friday pm. The Physician of the week will
also lead inpatient ward rounds at the Vale of Leven Hospital during
this week.
Weekend cover is provided by two Consultant Physicians with joint
morning ward rounds at the RAH and afternoon ward rounds at both
the RAH and VOL.
7.2
Coronary Care Unit
Patients with acute myocardial infarction, acute coronary syndromes,
arrhythmias or congestive cardiac failure at the RAH are admitted to
the 18 bedded coronary care unit, staffed during the day by one of the
consultant cardiologists on a rotational basis, supported by junior
medical staff from the cardiology ward.
7.3
Medical High Dependency Unit (HDU)
The RAH has three medical HDU beds at present within the combined
12 bedded HDU, with scope to increase to four or more medical HDU
beds if required.
7.4
Junior Doctors out-of-hours
A Hospital @ Night service started at the RAH in August 2006. This is
part of a wider work assessing the impact of modernizing Medical
Careers and implementing plans to support the delivery of high quality
sustainable services.
Junior medical support at VOL is provided by GPST1 and GPs
supported by the roll out of HAN Practitioner service.
8. Medicine for the Elderly Service
Medicine for the Elderly is part of the Rehabilitation and Assessment
Directorate, but many of its activities and adult medicine are combined.
In particular, out-of-hours cover is provided by juniors from Medicine for
the Elderly and adult medicine and both department share educational
meetings.
The Medicine for the Elderly Consultants visit the acute receiving ward
at the RAH on a daily basis and arrange transfer to the Medicine for the
Elderly Unit of those appropriate patients. A Stroke Unit (Ward 4) taking
all age groups is led by Drs L Erwin and Slavin.
The service at the Vale of Leven will support post-admission care of
appropriate patients with 37 rehabilitation beds which will also provide
near to home rehabilitation for patients admitted initially to the RAH.
9. Management Arrangements
Emergency Care and Medical Services Directorate – Clyde Sector,
consists of Emergency Medicine and General Medicine. The consultant
appointed will be expected to participate fully in the work of the
Directorate.
The Clinical Director for Medicine across Clyde is Prof Martin McIntyre,
based at RAH. There are two Lead Clinicians for Medicine across
Clyde: Dr Stuart Hood at RAH/VOL and Dr Chris Jones at Inverclyde
Royal Hospital.
10. Summary of the Post
The appointee will provide consultant leadership in Gastroenterology.
10.1
Duties of the Post
Inpatient: Gastroenterology beds based at RAH.
Outpatient: Clyde, based at Royal Alexandra Hospital with outreach to
either Vale of Leven or Inverclyde Royal Hospital.
Endoscopy: Clyde, based at Royal Alexandra Hospital with outreach
to either Vale of Leven or Inverclyde Royal Hospital.
10.2
Suggested job plan
The job plan will be tailored to the successful candidate’s
interests and the following should be taken as guidance. A detailed
job plan will in due course be agreed with the Clinical Director or
Associate Clinical Director and thereafter this will be subject to periodic
review and adjustment if necessary. The appointee may be required to
support clinical activity elsewhere across Greater Glasgow and Clyde.

Cover for study leave, holidays will be provided from within existing
consultants.
10.3
On call duties
The requirement for oncall including general medical commitments will
be agreed amongst the Gastroenterology team.
10.4
Support staff
There is secretarial support for the consultant.
10.5
Specialty Interests
The unit will try and support the candidate in development of any
specialty interest or technique in which they wish to participate.
11.
Postgraduate and Undergraduate Training
The unit has a postgraduate programme which includes a weekly unit
meeting, monthly journal club and SHO tutorial sessions. There are
weekly hospital postgraduate meetings during term time. Mr Andrew
Renwick is the postgraduate tutor.
The Medical Unit teaches a number of medical students from Glasgow
University. As part of the new curriculum teaching of years one to five
is becoming established.
