To be able to provide clear explanations of changes in patients vital signs. To understand the need to implement changes in care promptly and to recognise the need to update care plans whilst demonstrating an understanding of the patients needs and underlying condition. Planned Actions Resources Discuss vital signs with mentor and consider the possible complications that may present when carrying out these observations. Mentor support Provide immediate and ongoing care for identified patients whilst being supervised by mentor. Care Plan Documentation Report changes in condition and decisions related to these changes to mentor/supervisor. Implement appropriate changes to nursing care and document appropriately in patient notes/care plan. Time Equipment Multi-disciplinary team Evaluation On reporting changes in patients condition can demonstrate knowledge of possible reasons for change. Can provide feedback to mentor on changing care needs of designated group of patients. Able to demonstrate delivery of agreed care changes. Can provide clear and detailed Care Plans for care delivered.
