NHS Greater Glasgow & Clyde Emergency Care and Medical Services – Clyde Sector Consultant Physician with an interest in Respiratory Medicine Royal Alexandra Hospital and Vale of Leven Hospital This post of Consultant Physician with an interest in Respiratory Medicine will be based at the Royal Alexandra Hospital (RAH), Paisley, five miles from the bustling West end of Glasgow. The post is a replacement post for a retiring consultant. The successful candidate will integrate into a team comprising four Consultants to develop respiratory medicine and support acute medicine across both Hospitals. This will offer a real opportunity for the successful candidate to be involved in the development of a successful modern consultant led service across two Hospital sites. 1. General Information The Royal Alexandra Hospital (RAH) is situated in Paisley and provides acute health care services to Renfrew District (pop 205,000). The Hospital is a 968 bed District General Hospital (DGH) which opened in 1986 and is one of the largest and busiest non-teaching hospitals in Scotland. In addition the hospital is currently the base for provision of services such as medicine, surgery, orthopaedics, ophthalmology, paediatrics and the majority of maternity services to the population north of the Clyde. The RAH provides a wide range of DGH specialties with excellent support facilities. The hospital has a first class modern radiology department and services. All departments participate in undergraduate teaching and there is an active postgraduate educational programme. The hospital enjoys an enviable reputation for undergraduate teaching and is highly popular with training grades. There is a good medical library service. The Vale of Leven Hospital (VOL) is situated in Alexandria, an area of outstanding natural beauty and serving a population in excess of 88,000 largely from Dumbarton, Alexandria and Helensburgh. The Vale of Leven Hospital retains some unscheduled medical care led by the Consultant Physician team from the Royal Alexandra Hospital and is supported by GP and GP trainees. In addition there are inpatient medicine for the elderly beds, short stay surgical beds together with day surgery and a full range of diagnostic and support services. 2. The Medical Unit The Medical Unit at the Royal Alexandra Hospital has 186 beds. This includes a 18 bedded Coronary Care / Chest Pain Unit, an 18 bedded Medical Assessment Unit (MAU) and a 24 bedded Acute Medical Unit (AMU). The medical unit provides an acute service in general medicine with clinics in acute medicine, diabetes and endocrinology, gastroenterology, cardiology, asthma, respiratory medicine and haematology. There are 4 medical High Dependency Unit (HDU) beds within a 12 bedded HDU. The Vale of Leven Hospital medical unit has a Medical Assessment Unit, a 12 bed acute medical receiving unit (AMRU) with provision for monitored beds and a further 27 general medical beds. 3. 4. Respiratory Services 6.1 There are 24 inpatient beds provided in ward 11 at RAH for Respiratory Medicine with 15 further beds in ward 14 which is adjacent. 6.2 Staffing for Respiratory services in the RAH & VoL is four Consultant Physicians. 6.3 There are outpatient clinics in general respiratory medicine, sleep problems and difficult asthma. 6.4 There is a state of the art pulmonary physiology equipment at both RAH and VOL including body plethysmography at both sites and exercise physiology at VOL. These laboratories are staffed by 1.6 wte pulmonary function technicians. A sleep service is also provided on both sites 6.5 There are 2 full time respiratory nurse specialists providing a service for in patients and out patients enhanced by nurse specialists from Glasgow. There is a lung cancer nurse specialist 6.6 Day case and in patient bronchoscopy is carried out at both RAH and VOL. There is a weekly thoracoscopy list depending on clinical demand. Bronchoscopy using EBUS will start in 2012. 6.7 There is a weekly Clyde lung cancer MDT videoconference including representation from all three Clyde sites, oncology, radiology, pathology, palliative care and surgery. 6.8 All cardiothoracic surgery is referred to the regional WOS cardiothoracic centre at the Golden Jubilee Hospital. 6.9 There is a respiratory physiotherapist providing in-patient and out-patient physiotherapy services. 6.10 The department has access to community based pulmonary rehabilitation for all appropriate COPD patients 6.11 There are plans to introduce early supportive discharge in early 2012. 7. Acute Medical Services The team of Consultant Physicians at the Royal Alexandra Hospital have responsibility for acute medical services at both the Royal Alexandra Hospital and the Vale of Leven Hospital. 7.1 Acute Receiving Acute medical admissions are admitted to the acute medical unit (AMU) at the RAH, which is staffed by one of the consultants on a rotating ‘Physician of the Week’ basis Monday morning to Friday evening. Patients are often first assessed in MAU. Patients are triaged to the appropriate specialty wards if not for early discharge. The acute care physicians are based in MAU and AMU and provide leadership for acute receiving as well as sharing weekday daytime receiving duties. Acute medical admissions at the VOL will be admitted to ward 3 (AMRU), which is staffed by a consultant on a rotating ‘Physician of the Week’ 16 week basis Monday morning through to Friday pm. The Physician of the week will lead inpatient ward rounds at the Vale of Leven Hospital during this week. Weekend cover for both hospitals will be provided by two Consultant Physicians with joint morning ward rounds at the RAH and late morning/early afternoon ward rounds at the Vale. There is an afternoon post receiving ward round at the RAH. 7.2 Coronary Care Unit Patients with acute myocardial infarction, acute coronary syndromes, arrhythmias or congestive cardiac failure at the RAH are admitted to the 18 bedded coronary care unit, staffed during the day by one of the consultant cardiologists on a rotational basis, supported by junior medical staff from the cardiology ward. 7.3 Medical High Dependency Unit (HDU) The RAH has three medical HDU beds at present within the combined 12 bedded HDU, with scope to increase to four or more medical HDU beds if required. 7.4 Junior Doctors out-of-hours A Hospital @ Night service started at the RAH in August 2006. This is part of a wider work assessing the impact of modernizing Medical Careers and implementing plans to support the delivery of high quality sustainable services. Out-of-hours junior medical support at VOL will be provided by GPST1 and GPs supported by the HAN Practitioner service. 8. Medicine for the Elderly Medicine for the Elderly is part of the Rehabilitation and Assessment Directorate, but some of its activities and adult medicine are shared. In particular, out-of-hours cover is provided by juniors from Medicine for the Elderly and adult medicine and both department share educational meetings. The Medicine for the Elderly consultants visit the acute receiving ward at the RAH daily and arrange transfer to the Medicine for the Elderly Unit of those appropriate patients. A Stroke unit (Ward 4) taking all age groups is led by Dr L Erwin. The service at the Vale of Leven will support post-admission care of appropriate patients with 41 rehabilitation beds which will also provide near to home rehabilitation for patients admitted initially to the RAH. 9. Management Arrangements Emergency Care and Medical Services Directorate – Clyde Sector, consists of Accident & Emergency and General Medicine. The consultant appointed will be expected to participate fully in the work of the Directorate. The Clinical Director for Medicine across Clyde is Prof Martin McIntyre, based at RAH. There are two Lead Clinicians for Medicine across Clyde: Dr Stuart Hood at RAH/VOL and Dr Chris Jones at Inverclyde Royal Hospital. The Clinical Services Manager for Medicine at RAH and VOL is Mrs Helen Colvin and the General Manager for ECMS in Clyde is Ms Melanie McColgan both based at the RAH. 10. Summary of the Post The appointee will provide consultant leadership in respiratory medicine. 10.1 Duties of the Post Acute Medicine 1 in 8 Physician at both the RAH and VOL Inpatient General Medicine, Respiratory beds based at RAH Outpatient Based at RAH and VOL Bronchoscopy Based at RAH and/ or VOL 10.2 Suggested job plan The job plan will be tailored to the successful candidate’s interests and the following should be taken as guidance. A more detailed job plan will in due course be agreed with the Clinical Director or Lead Clinician and thereafter this will be subject to periodic review and adjustment if necessary. The one SPA minimum will reflect activity such as appraisal, personal audit and professional development occurring outside study leave time. Once the candidate has been appointed more SPA time may be agreed for activities such as undergraduate and postgraduate medical training which takes place outside direct clinical care, as well as research and/or management. These activities must be specifically and clearly identified and be agreed with the candidate and desired by the department. 10.4 On call duties The appointee will be expected to take a share with the existing Consultant Physicians of the general medical workload in terms of out of hours cover. The appointee would be expected to be on call every 10th weekend and be on call from home for advice for RAH and VOL every 20th night. 10.5 Support staff There is secretarial support for the consultant. 10.6 Specialty Interests The unit will try and support the candidate in development of any specialty interest or technique in which they wish to participate. 11. Postgraduate and Undergraduate Training The unit has a postgraduate programme which includes a weekly unit meeting, monthly journal club and SHO tutorial sessions. There are weekly hospital postgraduate meetings during term time. Mr Andrew Renwick is the postgraduate tutor. The Medical Unit teaches a number of medical students from Glasgow University. As part of the new curriculum teaching of years one to five is becoming established. NHS GREATER GLASGOW & CLYDE Statement of Policy regarding fitness to practice proceedings by a licensing/regulatory body and relating to criminal investigations in the UK or overseas. Registration with General Medical Council or General Dental Council imposes on doctors and dentists the duty to provide a good standard of medicine care for, and to behave appropriately, towards patients.
