Use this form to request permission to use the Personal Collections feature of
ARTstor.
NOTE: ARTstor Personal Collections can only be technically shared and viewed
within the University of Oregon community.

Terms of Use Agreement
By my signature, I confirm that:
a) my use of the uploaded materials falls within the Fair Use provisions of
the the Copyright Law;
or
b) the materials used are in the public domain;
or
c) I have permission from third-party copyright holders to place the
material in ARTstor;
and d) (if submitting student works) that I have written permission to place
student works in an ARTstor Personal Collection in accordance with FERPA
(Family Education Right to Privacy Act) regulations (see below);
and e) I will not knowingly upload any content that violates any intellectual
property, contractual or third party rights, or that violates any international or
domestic, federal, state or local law, statute or regulation;
and f) I will not knowingly upload any content that contains software, viruses
or any other computer code, files or programs designed to interrupt, destroy or
limit the functionality of any computer, software or hardware or
telecommunications equipment.
You also affirm that neither ARTstor nor the University of Oregon shall be liable
in any way for any content in Personal Collections that you have chosen to be
uploaded onto ARTstor’s servers, or for any use, display, performance,
reproduction, or distribution of such content. You further acknowledge that
ARTstor and its designees, or designated University of Oregon staff may at any
time remove from the ARTstor servers and disable access to any content that it
is believed that may violate the terms of section e) of the above Terms of Use
Agreement.
Student Work
If you are placing student work in ARTstor Personal Collections, you must have
on file written permission from that student. Student work from current or past
terms is protected under FERPA (Family Educational Right to Privacy Act). The
student identification number cannot be on the student work or attached data
placed in ARTstor Personal Collections. We recommend that the grade also not
be visible or present. (If the grade appears, you should document that the
student has been made aware of this before they signed the permission
