PHYSICIAN STANDING ORDERS FOR SPECIAL NEEDS SHELTERS
(For patients age 18 or older)
Patient Care
Vital Signs every shift. Notify on call physician for temperature greater than 100.4, Systolic blood
pressure less than or equal to 90 mmHg or greater than or equal to 160 mmHg, pulse less than or equal
to 50 or greater than or equal to 120, respiratory rate greater than or equal to 30 or less than or equal
to 8 and pulse oximetry less than or equal to 90%.
Resume patient’s home oxygen at patients home flow rate.
Accu-cheks on admission for all diabetic patients and thereafter every shift. Notify on call physician for
blood glucose greater than 250.
Resume patient’s home medications. Notify physician for all patients on anti seizure medications,
coumadin and psychotropic medications.
Notify on call physician for patients with chronic wounds for wound care orders
Medications
Ibuprophen 200mg: One to two tablets orally every 4-6 hours as needed for pain or fever.
Acetaminophen 500mg: One to two tablets orally every 4-6 hours as needed for pain or fever.
Benadryl 25mg: One to two tablets orally every 4-6 hours as needed for runny nose, insomnia, sneezing,
or itchy watery eyes.
Pepcid 20mg: One tablet every 12 hours as needed for heartburn.
Maalox suspension: 10 20 mL orally every 6 hours as needed for heartburn.
Loperamide 2mg: Two tablets orally times one then one table orally every 4 hours as needed for
diarrhea.
Kaopectate suspension: 30 mL orally every 2 hours for diarrhea.
Triple Antibiotic Ointment: Apply twice a day as needed for minor cuts, scrapes, and burns.
Benadryl Cream 2%: Apply to affected area three to four times a day as needed for relief of itching.
Hydrocortisone cream 1%: Apply to affected area three to four times a day for relief of itching
associated with rashes and inflammation.
Tums: Chew 2-4 tables as needed for heartburn.
