Protocol for the Transfer of Care of Respiratory cases on an out patient basis
To Neath Port Talbot Hospital
From September 2012 all acute inpatient facilities and acute medical Doctors to look after respiratory cases will
cease to be available and therefore no inpatient transfer of acute care will be possible.
Aim:
To ensure best clinical practice maintained and good communication and coordination between service providers
on all four sites for the safe and timely repatriation and follow up care at NPTH for this group of patients.
Patients from the Neath Port Talbot area who are discharged from Singleton, Morriston and Princess of Wales
Hospital may be followed up at NPTH if a discharge summary is immediately FAXED to Dr Ebejer/Dr Vardills
secretaries. Tel. 01639 862038; FAX (awaiting new fax machine)
1) The Respiratory Nurse follow-ups patients with pneumonia, Tuberculosis, Bronchiectasis, unstable
asthmatics and COPD patients residing in the NPT locality.
2) We do no routinely follow-up uncomplicated asthma or COPD.
3) New Respiratory referral requests to Respiratory Consultants.

4) The following patients are also followed up in Nurse led clinics: Tuberculosis, Bronchiectasis, COPD, Asthma
and nebuliser trial clinics.
5) The Respiratory Nurse receives referrals from Public Health for Port of Entry screening for cases residing in
the NPT locality. Contact tracing for other sites of Tuberculosis cases that have contacts that wish to be
screened at NPTH.
6) Currently Respiratory patients that are known to the chest Consultants/Respiratory Nurse at NPTH are able
from 8am- 4pm Monday to Friday contact the Respiratory nurse direct for advice on management in the
event they are becoming unstable. (Thus putting in place appropriate management plan dependant on
Respiratory problem and initiating treatment out in primary care where possible.) Tel 01639 862275 –

7) Based on outcome of point 5 patients are then seen within same week or 2 weeks for review in the
Respiratory Nurse clinic. (appointments are expedited)
Patients who may be suitable for pulmonary rehabilitation and who have been assessed by a Respiratory
physician/Respiratory Nurse may be referred directly to the COPD team.
