Respiratory Linkage aims to educate and support clients to effectively manage their
respiratory condition. With effective management disease progression may be slowed and
hospital admissions reduced.
Appropriate linkages and referrals to other healthcare and service providers ensure a holistic
approach to condition management and promote client independence.
Clients are individually reviewed by a respiratory nurse specialist. Reviews may be in a clinic
setting (Joondalup or Osborne Park), at home or by phone at the discretion of the respiratory
nurse.
Review is in accordance with current guidelines and may incorporate:

History taking
Spirometry
Condition education
Respiratory medication review and education.
Inhaler technique
Pneumonia and influenza vaccination status
Review and education regarding oxygen therapy
Recognition of exacerbations and development of an Action Plan (in consultation with GP or
respiratory physician)
Coping strategies
Promotion of pulmonary rehabilitation
Anxiety and depression score (PHQ-9)
Liaison and /or referrals
This service runs Monday to Thursday
There is no cost
Referrals accepted from Health Care Professionals
Eligibility
Confirmed respiratory condition:

o
o
Chronic Obstructive Pulmonary Disease (FEV1 <60% predicted)
Bronchiectasis (not Cystic Fibrosis)
Interstitial Lung Disease
Living in Black Swan Health catchment.
