INTERVENTIONAL CARDIOLOGY (IC)
This questionnaire is to provide the Royal College with a complete description of the AFC program. The
completed questionnaire must be signed by the AFC director and submitted to the office of the Assistant
or Associate Dean for Postgraduate Medical Education for the approval of the Faculty Postgraduate Medical
Education Committee.
