G1 Student Field Contract between Youngstown State University ______________________________ (agency name), ______________________________ (student name), Youngstown State University considers field instructors a vital part of the education of social work students and recognizes the importance of the contribution of the agencies where these students are placed. To maintain the educational excellence desired by all parties in this joint program, a specific statement of the responsibilities of the three parties is listed in the following paragraphs. Responsibilities of Youngstown State University 1. The YSU Social Work Internship Coordinator will provide the agencies with a manual setting objectives, policies, and content of field instruction. 2. In cooperation with the agencies, the YSU Social Work Internship Coordinator will assign the students to be placed in the agencies. 3. The YSU Social Work Internship Coordinator will provide the agency with background information on the student(s), including any prior convictions or limitations to field. 4. The YSU Social Work Internship Coordinator will assist the agency in planning an educational program for the student, and will be available for consultation whenever problems occur. Visits to the agency by the YSU Social Work Internship Coordinator will take place at least twice the first semester of placement and once the second semester of placement. 5. The university and the YSU Social Work Internship Coordinator will provide seminars, workshops and meetings for agency field instructors and administrators. 6. The YSU Social Work Internship Coordinator has the responsibility for evaluating the overall achievement of the student and will assign the grade. This will be based on the agency field work instructor's evaluation of the student's performance. 7. The YSU Social Work Internship Coordinator will send a Faculty Data Form to the agency field instructor. The YSU Social Work Internship Coordinator must also sign the Field Work Contract and Learning Contract for the Field. 8. Youngstown State University carries liability insurance for each student during the field work placement. YSU maintains professional liability insurance in the amount of $1,000,000.00 per occurrence and $3,000,000.00 in the aggregate per year for its students and its faculty present at the agency. G2 Responsibility of the Agency 1. The agency will provide a field instructor who is an agency employee and who has a master's or bachelor’s degree in social work and who has the knowledge and experience the university considers essential. OR 1a. If the agency does not have a qualified social worker, it may wish to contract with a qualified social worker to provide supervision to YSU students. Under these conditions, the agency works closely with the Internship Coordinator to insure an ethical, competent, and meaningful experience for student. 2. The field instructor will be allowed a minimum of one hour agency time weekly per student for individual conferences. Additional release time must also be granted for case assignments and monitoring, reading student recording and reports, and any other tasks associated with field learning. It is also expected that the field instructor(s) workload will be adjusted in order to assure adequate time to meet the learning needs of students. 3. The agency will make available to the student, through the field instructor, learning experiences which are suitable in quality and quantity for the student's maximum professional growth. 4. The field instructors will accent, as their teaching objectives, the learning objectives mutually agreed upon by the university, agency and student. 5. Students should be involved with the field instructor in an ongoing evaluation of field performance. 6. It is the primary responsibility of the field instructor to report in written form an evaluation of the performance of the student(s) to the university at midterm and at the end of each semester. A verbal conference on the student's performance will be held each semester with student, field instructor and YSU Social Work Internship Coordinator present. 7. The field instructor will help plan and will attend seminars, workshops and meetings pertinent to field instruction. 8. The agency field instructor must complete the Field Instructor Qualifications Form and submit it to the University. The agency field instructor must also sign the Field Work Contract and the Learning Contract for the Field. G3 Responsibilities of the Student 1. The student will complete the application for field work, submit a chronological resume and agree to a pre-placement interview at the placement agency. If the field placement agency and student agree to the placement, a finalization conference with the student, agency field instructor and YSU Social Work Internship Coordinator will be held. The student must sign the Field Work Contract and the Learning Contract for the Field. 2. The student will abide by agency rules, policies, and procedures, including maintaining of client confidentiality, appropriate dress and attendance. 3. The student will abide by the NASW Code of Ethics. 4. The student will be responsible to the field work instructor and perform assignments under his/her direction. 5. The student is responsible for taking an active part in his/her learning. This includes being prepared for supervisory conferences, completing required recording and reading assignments given by the field work instructor. 6. The student will discuss questions, problems or complaints concerning the field placement first with the field work instructor and then, if unsatisfied, with the YSU Social Work Internship Coordinator. 7. The student must complete the Student Evaluation of Field Work and Seminar forms and return them to the YSU Social Work Internship Coordinator on the designated date. 8. Student is responsible for being aware that they will not be considered as employees of the agency and will not be covered by its workers’ compensation, malpractice or other insurance coverage. G4 The parties here to recognize their interdependence and recognize that each will derive the greatest benefits by promoting each others interests. Each of the parties is, therefore, entering into the agreement with the intention of cooperating with the other in carrying out the terms of the agreement, and agrees to interpret its provisions in such a manner as to best promote the interests of social work education, the client and the community. This agreement will be reviewed periodically and may be altered or modified by mutual consent.
