Brahmananda Narayana Multispeciality Hospital exemplifies its expertise in Paediatric Cardiology Identifies congenital birth defect in a 15 year old girl being the first in Jharkhand Uses Jugular vein from the neck to perform the cardiac procedure instead of the usual method Jamshedpur, October 17, 2014: A team of paediatric cardiologists at Brahmananda Narayana Multispeciality Hospital successfully performed a unique cardiac procedure on a 15-year-old girl who suffered from Patent Ductus Arteriosus (PDA), an extremely rare case - found only in 1 amongst 10 lakh children. She was the first patient in Eastern India to be treated for such a rare form of congenital disorder of the heart. Brahmananda Narayana Multispeciality Hospital is the only center in Jamshedpur to have expertise in paediatric cardiology speciality, with a team of experts treating children born with heart defects. The hospital now has a full-time paediatric cardiologist also available. Jyotsna, the young patient from Ghatsila, Jharkhand was brought to Brahmananda Narayana Multispeciality Hospital, with severe symptoms of recurrent cough, cold and was not gaining weight. Investigations revealed that that she was suffering from Patent Ductus Arteriosus (PDA), a congenital disorder of the heart where a normal blood vessel that connects two major arteries supplying blood from the heart to the developing foetus, fails to close after birth. This is a normal foetal blood vessel in all babies that closes soon after birth. But, in this case it had not closed. Speaking on PDAs, Dr Pankaj Kumar Gupta, Consultant Paediatric Cardiologist said “Patent Ductus Arteriosus, is a highly rare condition which makes timely diagnosis a big challenge. Early symptoms are uncommon, but during the first year of life, there is difficulty in breathing and poor weight gain. PDA is common in patients with persistent respiratory problems such as hypoxia, and has a high occurrence in premature children. With age, PDA if untreated may lead to heart failure.” Talking about the case, Dr Amitabha Chattopadhyay Senior Consultant-Interventional Paediatric Cardiologist said “This is one of the most unique cases we have had, and also this procedure is the first that is done not only in the state of Jharkhand, but in East India. Through continuous follow ups and medication, she is doing very well and is resuming her normal life. She cancontinue her studies and do everything just like any other normal adolescent.” Jyotsna’s case of PDA was treated through a very unique procedure termed as - PDA device closure through the Jugular vein in the neck. Usually, the procedure involves the PDA getting closed by using a device which is made up of a wire (Nickel and Titanium alloy), usually imported from America. This involves the device being introduced through the vein of the leg. With the help of delivery cables and catheters, the device is carried to the heart and after complete occlusion it is released, which then closes the blood vessel. In Jyotsna’s case, it was found that both the leg veins were abnormal. The procedure could not be done through the normal way i.e., from the leg veins. The alternatives were either surgical closure or the device to be delivered through the neck veins or through the aorta. Due to the large PDA, the aortic route was unsuccessful leaving to deliver the device through the Jugular vein in the neck as the only option. The method is very challenging and needs highly skilled expertise in cardiac procedures to perform. The procedure was successful, being the first in the state of Jharkhand. Two days later, the girl was discharged in a stable condition.
