2015-2016 Continuous Improvement Plan Summary

At Worthington Hills Elementary, we are committed to nurturing students academically, socially, and emotionally
to support their ongoing growth and success. Our school team is working to support the following district goals for
continuous improvement as evidenced by the action steps and strategies listed below each goal area.
District Goal #1: Ensure that each student feels physically and emotionally safe and feels connected to the
school and school staff as evidenced through PBIS plans and monitoring data, student surveys, discipline and
attendance data, focus groups, etc.

Refine and fully implement our PBIS plan with focus on decreasing the number of 2014 – 2015
referrals regarding students being bullied.
a. The PBIS (Positive Behavior Interventions and Support) team will survey the students in
grades 2-6 to address school-wide expectations involving bullying. Then analyze the results
and provide guided instruction for school wide expectations.
District Goal #2: Maximize academic performance and growth for each student as evidenced through
appropriate data to include increased enrollment in high rigor coursework (Honors, IB, AP, CC+) and
improved academic performance on identified assessment/performance data (PI, MAP, grades, SAT, ACT, AP,
IB, Compass, etc.).
1. Fully implement the Building Leadership Team and Teacher Based Team Structures, ongoing
monitoring of student work and student achievement evidenced through the 5-step process.
2. The Worthington Hills staff will create two opportunities for the teacher, parents, and the students
to discuss and develop student goals that will impact student achievement.
a. Each staff member will analyze their own data/assessments to determine with each
individual student a targeted area that the student, teacher, and parent can collaboratively
work on to achieve the student’s goal.
District Goal #3: Ensure that students graduate remediation free for entry into post-secondary education and
careers (college and career).

Implement an RTI framework which monitors the growth of students to ensure growth for all so
100% of students below grade level will exceed their annual reading and math RIT goal as measured
by NWEA MAP. Then 100% of students at or above grade level will reach their annual reading and
math RIT goal as measured by NWEA MAP.
a. All teachers will be explicit with their learning targets, feedback, and student goal setting to
increase student ownership and engagement in their learning.
