Step 4: Step by step guidance on completing Individual Provision Map/Individual Education Plan
All children are required to be on Early Years Action Plus (EYA+ Low / Medium / High)
A child moves from Early Years Action (EYA) to Early Years Action Plus (EYA+) because specialist agencies are involved with the child to support a specific aspect or
aspects of their development.
The initial targets and strategies provided by the agency are discussed with the parents/carers.
It is important to record the different discussions with the parents/carers as the SEN Code of Practice states that they should be central to their child's care and any
individually planned provision.
The form Minutes of Review of Intervention (Form a) can be used as it records the parents/carers views and it should be signed and dated.
The agreed strategies and targets and are included within an Individual Provision Map/Individual Education Plan (4b).
We advise settings to use Individual Support Plans for children under the age of 3 (see document 4d) to detail the general management strategies used to support
the child. For complex children it may be appropriate for an IEP/IPM to run alongside the ISP.
(Whilst there are different names for the different formats, the content of the forms are more important than the title of the paperwork.)
The Individual Provision Map/Individual Education Plan/Individual Support Plan is to be reviewed approximately 6- 8 weeks with the parents/carers and outside
agencies if appropriate. These discussions should be recorded on the Minutes of Review of Intervention form (4a) and signed by the parents/carers.
Newly agreed targets are included within a revised Individual Provision Map/Individual Education Plan/Individual Support Plan (4b).
The Record of Intervention can be found on the back of the Individual Provision Map/Individual Education Plan/Individual Support Plan (4c). This must be on-going
as it provides evidence of the intervention. This is essential to support the Worcestershire Pre-School Forum in understanding the progress made over a period of
time.
