Post Heart Transplant Nurse Coordinator

Will provide direct care to short and long term post Heart Transplant patients with
multiple, often complicated acute and chronic medical problems. Will also coordinate
multiple post transplant, in hospital, and outpatient services for the efforts of the
Transplant Surgery Team, Consulting Services, and the nursing care as pertains to the
post Heart transplant patient.
Will provide ongoing education to the post transplant patient (including patient’s family
and primary care giver), both inpatient and outpatient, and working with re-admitted
patients to deal with ongoing medical issues and manage logistics of donor offers for
Heart and/or Lung transplant patients during regular working hours. On call commitment
may be required on evenings, weekends and holidays.
Requirements
-BSN degree
-Current CA RN License
-Current AHA CPR certification
-5+ years cardiopulmonary critical care experience
-The flexibility to orient and work at all UCSF Medical Center locations

-CCTC
-MSN degree
-Previous transplant coordinator experience
