Here’s your first assignment in our partnership for helping you to reach the goal of a Kidney Transplant!
If you follow the steps in this letter, you will be seen for a Transplant Evaluation visit.
If you miss any of the items listed below in #1-6, you will be asked to reschedule for another time.

KEY STEPS YOU NEED TO COMPLETE:
ONE WEEK PRIOR
1) Call our office at 302-623-3866 to speak to a Transplant Coordinator
(The coordinator will obtain information about your medical history)
DAY OF EVALUATION
2) Please arrive promptly at 7:45am so we can get you checked in and start the education session on time
3) Bring all your Insurance Cards; this includes both your MEDICAL and PRESCRIPTION insurance cards
4) Bring a Written List of Medications- Name, dosage (i.e. 11mg) and how often you take it (daily, twice daily; etc)
5) Bring a Written List of your Physicians & Phone Numbers (i.e. Family Doctor, Nephrologist, etc.)
6) Bring a Written List of your Contact Info (Name, Phone Numbers, Email, Emergency Contact & Number)
STRONGLY RECOMMEDED FOR DAY OF EVALUATION
7) Eat a full breakfast, pack a lunch or bring snacks since you will be in our office for several hours (7:55am – 2:00pm)
8) Bring a Family Member or Friend with you. This person will help you understand and remember information you
received during the Evaluation visit, AND this person can potentially help with living donor recruitment.
