Nurse Practitioner (1.0 FTE) – Kincardine FHT
The Kincardine Family Health Team will provide comprehensive patient care utilizing an
interdisciplinary approach that is responsive to the needs of the community. We are guided by our
commitment to excellence and leadership through quality care with a focus on health promotion and
disease prevention. We endeavour to provide residents of the Municipality of Kincardine and
Township of Huron-Kinloss access to quality care and to be recognized as a leader in the provision
of quality rural healthcare by providing continuity of care and expanded access to health services.

POSITION SUMMARY
The Nurse Practitioner will work within primary care and the scope of practice of a Nurse Practitioner
as outlined in the CNO standards of Practice for Registered Nurses in Extended Class.
MAIN DUTIES AND RESPONSIBILITIES

Individual patient assessment and treatment

Assisting the FHT with program development to meet the needs of our patient population

Promoting of quality improvement initiatives which enhance care delivery processes and patient
outcomes within the FHT

Supporting inter-disciplinary learners as they rotate through the clinic

Function as the Cardiac Rehabilitation Team lead

Other duties as assigned as our clinic grows and evolves

Other duties and expectations to be discussed with successful applicant
QUALIFICATIONS AND EXPERIENCE

Current College of Nurses of Ontario registration, as Registered Nurse (Extended Class)

Minimum 2 years relevant experience as a licensed Nurse Practitioner

Previous Cardiac Rehabilitation experience highly preferred

Strong computer literacy is required to facilitate the use of electronic medical records

Strong organizational, interpersonal, and communication skills

Ability to work independently and cooperatively in a busy multidisciplinary team
