DOCUMENTS NEEDED TO PREPARE ORDER
If you have the following documents in your possession, you can either scan and
email or mail them to me directly at the address listed above:
 Draft or signed copy of any property settlement agreement; and
 Final Decree of Divorce.
Please contact either your human resources department or plan administrator and
request that the following documents be emailed or mailed to be directly:
 Summary plan description for all retirement plans;
 Any available instructions for the preparation of a retirement order; and
 Any models used by your retirement plan in the preparation of a retirement order.
The important dates for the preparation of retirement orders are the date of
marriage and the date of separation. If all of the retirement plan benefits were accumulated
during the marriage, I do not need a statement of your benefits as of the date of marriage.
However, I will need a benefit statement as of the date of separation. If a part of the retirement
plan benefits were acquired prior to the marriage, I will need a benefits statement as of the date
of marriage and a benefits statement as of the date of separation.
If you do not have or cannot obtain any of the above documents, please
notify me immediately. However, most employers have a section on their
benefits website that can provide you with this information.
