Dear Parents/Carers,
After School Sports Activities
We are requesting parents return the form below to indicate the after school club their child
is interested in attending. As the clubs are usually oversubscribed children will be selected
fairly based on who did not take part last term. The lessons are grouped for the different
age ranges, all lessons will take place from 3.30 – 4.30pm starting the week beginning 13th January.
Please tick the sport your child is interested in attending and return by Thursday 9th Jan.
